Before you begin the application, have ready copies of your personal narrative, resume, and if available, copies of transcript(s) to upload; and payment information for the application fee.
Application fee of $75 is due with application and is paid after you submit your application.
Prepare a personal narrative, not to exceed four (4) typed pages in length (double spaced, 12-point font, and one-inch margins). For this narrative, respond to ALL the items listed below. This narrative is a critical part of your application packet. Please utilize APA style to format the paper and cite sources (if necessary).
Part One: Explain your interest in the CSSWP. Elaborate on both personal and environmental factors that have influenced your decision to pursue this program. This can include any life experiences that have influenced your decision.
Part Two: Discuss your understanding of the Sport Social Work field. Describe the skills you believe a successful Sport Social Worker must possess.
Part Three: Please state whether you will pursue an LCSSW after the CSSWP program
Part Four: Demonstrate self-awareness by discussing your strengths and weaknesses as it relates to working with the athlete population. How do you believe this certificate program will enhance these strengths and address areas of weakness?
Please submit a current copy of your resume. Your resume should outline 1) all professional work and 2) sport volunteer experiences.
